Call the automated hotline number and provide your contact information and date of birth will place you in a queue to receive a call back for scheduling.
Every eligible individual will receive a call back from a representative to schedule a vaccine appointment.
Arrive on time for your vaccination appointment. Remember to bring a form of ID, proof of residency and CDC appointment card if this is your second dose.
