Eligibility Requirements

Florida Department of Health in Okeechobee County

Fees for service are based on income and family size on a sliding fee scale. All clients, regardless of income are required to pay full cost for certian services, such as Vital Statistics or International Travel Immunizations. Fee schedule is available upon request.

Be sure to bring the following with you:

  • Drivers License or Valid Identification (ID)
  • Proof of Income (Last 4 paycheck stubs)
  • Medicaid Card
